Medical Records Requests
To request a complete copy of your child’s medical records, please submit the Medical Records Release form below. A separate form is required for each child. Patients age 18 and older must complete and submit their own authorization.
There may be a fee for processing of records as defined in NJ State Law Section 1 of P.L.2019, c.217.
SHOT RECORDS
Need Vaccination Records?
A medical records release form is only required for complete medical records. For vaccination (shot) records, please call the location nearest to you. Please note that it may take up to 5 business days to complete your request, as all records must be reviewed by our nursing staff. Processing times may be longer during periods of high demand.
